By removing the top cog and inserting a spacer for that amount on the low side, the clearance between the rear derailleur and the spokes is increased, which prevents the rear derailleur from getting caught in the wheel and breaking. This is particularly effective in cyclocross races.
While it reduces the number of cogs by one (from 11s to 10s, or 10s to 9s), even professional riders rarely use the top cog in races. This is because the inner-low combination significantly loosens chain tension, leading to more problems, so it's almost never used. Also, unless it's an extremely high-speed course, the outer-top combination isn't used either, so you're unlikely to notice any disadvantage from having one less cog.
